FUOtuoke Supplementary Post UTME Form Information for 2024/2025
The Federal University Otuoke invites applications from interested candidates for Supplementary Admission into the 2024/2025 Academic Session. Interested candidates are to be guided by the following:
ELIGIBILITY
Candidates who chose the Federal University Otuoke as their first choice and scored 140 and above in the 2024/2025 UTME and are ready to study any of the under-listed programmes.

- Candidates who did not choose Federal University Otuoke as their first choice but scored a minimum of 140 in the 2024 UTME and are ready to change to Federal University Otuoke as their first choice are also eligible to apply. Candidates in this category must do a change of institution to Federal University Otuoke via JAMB website (www.jamb.org.ng) and choose any of the programmes listed above.
- Candidates who initially applied for admission at Federal University Otuoke but were not admitted into the programmes of their choice can also reapply for any of the programmes listed above but must first do a change of course to the desired programme at JAMB Website.
- Change of Institution /Course at JAMB may take 24hours to be updated.
METHOD OF APPLICATION
Candidates are to make a Non-refundable payment of Two Thousand Naira (N2,000.00) only by using an Interswitch Verve enabled or Master card for the application form.
DOCUMENTS REQUIRED FOR THE ONLINE APPLICATION
Candidates are required to upload the following documents:
- Passport-size photograph
- Copy of 2023 UTME original result printout
- Copy of WAEC/NECO/NABTEB Ordinary Level result
- Copy of Local Government Origin
- Copy of Birth Certificate or Age Declaration
SPECIAL INSTRUCTION
- Use digital passport photograph
- Use the same name as in your JAMB UTME and in the same order
- Submit a valid email address and personal phone number in your online application
- Note: Federal University Otuoke will not be responsible for unsuccessful applications as a result of invalid email address(es) and telephone number(s).

The following categories of JAMB UTME candidates are eligible to apply:
a. Candidates who selected the Federal University Otuoke as their first choice in the 2024/2025 JAMB UTME and chose any of the programmes listed below and scored a minimum of 140.
b. Interested candidates who did not choose Federal University Otuoke as their first choice but wish to study in Federal University Otuoke and scored a minimum of 140 in the 2024/2025 JAMB UTME.
Candidates in this category must do a change of Institution to Federal University Otuoke via JAMB website (www.jamb.org.ng) and chose any of the programmes listed below.

- Digital Passport-size photograph
- Scanned copy of WAEC/NECO/NABTEB O’ level result (in PDF format)
- Scanned copy of LGA identification letter (in PDF format)
- Scanned copy of Birth Certificate or Age Declaration (in PDF format)
SPECIAL INSTRUCTION
a. Use digital passport photograph (in JPG format)
b. Submit a valid email address and personal phone number in your application
c. Upload your O’ Level Result(s) scratch card information. For candidates combining two O’ Level Results, both scratch card information should be uploaded accordingly.

COMMENCEMENT OF ONLINE APPLICATION: The Online application will be open from Thursday 25, July 2024 on the University’s portal http://ecampus.fuotuoke.edu.ng.

Online application portal closes on August 18th, 2024.
FUOTUOKE POST UTME SCREENING DATE
The date for the Post-UTME Screening Exercise would be made known upon completion of the application, on the PUTME Screening Schedule Printout.
DISQUALIFICATION
CANDIDATES WHO MAKE FALSE DECLARATION OF UTME SCORES, ORDINARY LEVEL RESULT GRADES (WAEC, NECO OR NABTEB) AS WELL AS STATE OF ORIGIN SHALL NOT BE CONSIDERED FOR ADMISSION.
Successful candidates will be issued letter of invitation to report at the university campus for further screening/verification.
