3. Collect your cash receipt from the account office
4. Proceed to the Coordinator's Office to collect application form

Step 3: Form Submission

2. Attach photocopies of your credentials (O'Level results, birth certificate, etc.)
3. Return completed form to the Coordinator's Office, School of Audiology
